Avon, Greater London, Greater Manchester, Cleveland, West Midlands, and Yorkshire split into the four seperate areas of Yorkshire (for PayPal compatibility) are included.

This file, to the best of my knowledge, covers every county listed in both the uk_counties_sql_script_for_royalmail_1.1 and uk_zones_paypal_compatible_1-0 contributions.

Tested and found to work in my store with version 1.3.8a of Zen Cart.

To install simply copy the contents of the All UK Counties.sql file, which should be included with this readme, and then paste the contents into Tools -_ Install SQL Patches in the admin interface of your installed Zen Cart setup.
