Pick up Delivery Date and Time

Domestic or local customers often need to select delivery date and time. Zen Cart does not has such function or module to do this. This module is created for one of my clients who live in United States. Here's requirements he need:
I need a drop down box asking what date they want it delivered.

It also needs to ask what time the delivery should be made (between 12pm and 7pm).

Lastly it needs a warning that only one delivery attempt will be made, and a box for a phone number to reach the person at before the delivery attempt is made.

All of these options need to show up on the orders page when viewing the details of the order.

Functions:

1. select delivery date by click a calendar box
2. select a specific time from a dropdown list, like 08:00, 09:00, 10:00...23:00
3. a phone number input field (optional)
4. Date, Time, Phone number will be displayed in Admin->Orders
5. Date, Time, Phone number will be displayed in Shop->My Account->Order History
