This is a pretty easy job for someone who knows what they're doing!

I have found a survey that I would like to put onto my website. This page would not be accessible from links on the main site but would be sent as a link to customers who have completed an order.

The html for the survey I want is on this page: http://www.logicaltips.com/LPMArticle.asp?ID=83.

I am looking for someone to add the survey onto my site. I wouldn't want the questions changed as I could change these once the survey is up and running. When the survey is completed, the results will be sent to my email address. If anyone is interested I would be grateful if you could send me a quote for the work, the time-scale for completion and, your experience in coding (including sites you've worked on for example).
